What We Do

Congratulations to Health Connect Corp. (December)
Health Connect Corp was the recipient of the December Game Changer Grant from Forward Community Investments. Every month FCI awards a Game Changer grant to a nonprofit organization or program in the state of Wisconsin that is doing outstanding or noteworthy work to reduce racial and socioeconomic disparities.

Health Connect is a non-profit organization seeking to help match people in the community with the right healthcare providers. HealthConnect is an online service that provides people the opportunity to search for healthcare providers that are in close proximity to them. Not only does it locate these providers, it also displays their availability to see new patients. The web search tool allows individuals to search for nearby providers for services including Primary care, mental health, dentistry, emergency housing, food pantries and childcare.
